Social Worker Duties:

• Evaluate client needs and contribute to creating Individual Care Plans alongside the IDT.  
• Provide counseling, support, and crisis management as necessary.  
• Collaborate with external resources to ensure comprehensive support for patients and families.  
• Visit clients’ homes in mountain areas, operating from the Evergreen office.  

Social Worker Requirements:

• MSW required; licensure preferred.
• Expertise in case management and counseling within healthcare settings.  
• Ability to participate in an on-call rotation.  
• Valid driver’s license, reliable vehicle, and appropriate insurance.
