Your Impact:

• Deliver premier primary care to a vibrant geriatric population in a welcoming outpatient setting.
• Provide compassionate, patient-centered care for residents in Assisted Living and Memory Care facilities.
• Lead care coordination efforts and facilitate goals of care meetings to align treatment with patient and family objectives.
• Perform comprehensive assessments for admissions to the Sub-acute Rehabilitation Unit, ensuring seamless care transitions.
• Be a key contributor to weekly Care-plan meetings, driving high-quality, integrated care.

What We Offer:

• Competitive base salary ranging from $236,500 to $300,000

Qualifications:

• Board-Certified Physician, with a preference for those with Board Certification in Geriatrics.
• Extensive experience in a Senior Living community, ensuring you understand the unique needs of older adults.

• Compassionate and patient-focused, with a natural ability to inspire trust and collaboration.
• Strong verbal and written communication skills paired with exceptional critical thinking abilities.
