Social Worker Responsibilities

• Assess needs and participate in Individual Care Planning with the IDT
• Provide support services and crisis intervention as needed
• Coordinate services for the patient/family with collateral resources
• Travel to homes in the foothills from Evergreen office base

Social Worker Requirements

• MSW required, license preferred
• Skilled at counseling and case management in healthcare
• Ability to participate in on-call rotation
• Valid Driver's License, Insurance and reliable vehicle
